Informed Consent and GDPR Information

Professional informed consent explains the nature of the service, its benefits and risks, the limits of confidentiality and the beneficiary's choices. GDPR information separately explains how personal data is processed.

The public form's independent checkboxes separately confirm that the online policy has been read and that the terms have been accepted. They do not constitute professional informed consent. The documents below are discussed and signed physically at the practice; online signing is not included in this version.

The signed document is kept in the professional file. The beneficiary may request a copy and verbal or written clarification. For couples, each adult signs separately. For minors, the rules concerning both parents and documented exceptions apply; designating a primary contact does not remove the other parent's legal rights.
